What is a Home Care Package?

The Home Care Packages program is a government-subsided program that provides long-term support for older people who want to stay living at home.

There are four levels of Home Care Packages for different levels of care and support needs. Each level receives a different amount of funding. This money can be used to buy hours of care or other support that suits your needs.

Home Care Package aren’t paid to you directly. Instead, you choose an approved Home Care Package provider (Wyanga) to administer the payments on your behalf to provide you ‘case management’ support and advice.

Once you have a Home Care Package, you can keep it for as long as you need. If your needs increase, you may be re-assessed for a higher level package.

Am I eligible?

To be eligible for a Home Care Package at Wyanga, you must be:

  • Aboriginal and/or Torres Strait Islander
  • Aged 50+
  • Suffering with a chronic medical condition

Application process

Apply through ‘My Aged Care’

  • Ask them for an Aboriginal assessor
  • Tell them you are interested in Wyanga

The following will then occur:

Assessment (ACAT)

  • Will be carried out to determine your needs and the appropriate level of care

Please note: it may take them a few weeks to arrange this

Wyanga staff co-ordinator visits your home

  • To develop an individual care / service plan
